PACE of Southwest Michigan is pleased to announce that Candace Graham, MSN, RN has joined PACE as Chief Clinical Officer. Candace brings more than 25 years of experience in healthcare to the position. “I have always valued the PACE model of care which is centered on the belief that it is better for the well-being of seniors with chronic care needs to be served in their home and community whenever possible,” said Graham . “Now, I get to take an active part in serving seniors and their families in our community.”

Graham received her Associate of Applied Sciences in Nursing from Ferris State University in Big Rapids, Michigan; her Bachelor of Science in Nursing at the University of Michigan, Ann Arbor; and her Master of Science in Nursing at Ferris State University.
She came to PACE from her position at Spectrum Health Lakeland Hospital in Niles, Michigan as Chief Nursing Officer. Graham also served as the Director of Care Management at Spectrum Health, Lakeland and as the Manager of Bronson Home Health Services, Kalamazoo, Michigan.
Graham is a member of the Michigan Organization of Nurse Leaders, American Case Management Society of America, and the Case Management Society of America
“This position allows me to use my experience in acute care, rehabilitation, care management, and nursing to provide oversight and support to the interdisciplinary team. I am thrilled to work with a team that is passionate in serving the geriatric population. Seeing and interacting with the participants validates that we are making a direct impact on someone’s life,” Graham commented .
